En Bloc Bipolar Prostate Enucleation Using the Mushroom Technique with Early Apical Release: Short-Term Outcomes

Summary

This study describes a new surgical technique for treating enlarged prostates that cause urinary problems. Instead of using expensive tissue-removing machines, surgeons used a simpler method inspired by mushroom shapes to safely remove the enlarged prostate tissue. The results showed that patients experienced significant improvements in urinary flow and symptoms, with most side effects being temporary and manageable.

Background

Transurethral resection of the prostate (TURP) remains the gold standard for benign prostatic hyperplasia (BPH) treatment, though anatomical endoscopic enucleation of the prostate (AEEP) offers a size-independent alternative. The mushroom technique provides an alternative adenoma removal method without requiring expensive morcellators, making it more accessible to various medical centers.

Objective

To evaluate the efficacy and safety of en bloc bipolar prostate enucleation performed exclusively via sheath-tip mechanical dissection combined with the mushroom technique and early apical release for treating BPH with significant lower urinary tract symptoms.

Results

Median operative time was 40 minutes with enucleated tissue weight of 34g. Significant improvements were observed in International Prostate Symptom score, Quality of Life score, maximum flow rate, and postvoid residual urine at 12 months (p<0.001). Transient stress urinary incontinence decreased from 19.44% at 1 month to 2.38% at 12 months with manageable complications.

Conclusion

En bloc bipolar prostate enucleation using the mushroom technique is a safe and effective treatment for BPH, yielding significant improvements in urinary symptoms and flow rates without requiring expensive morcellators or dedicated enucleation loops, though longer follow-up studies are needed to confirm long-term durability.
